Free Dementia Support Through the GUIDE Program

Lone Star Neurology is partnering with Craniometrix to offer the GUIDE (Guiding an Improved Dementia Experience) program — a free Medicare-funded support service for patients living with dementia and their families. Once your provider makes a referral, a dedicated Care Navigator will work with you to create a personalized care plan, provide 24/7 helpline access, and connect you with local services and caregiver support — all at no cost to you.
