What Is Essential Tremor?
Essential tremor is widely misunderstood – yet it is, in fact, the most common movement disorder in the world. It is characterized by involuntary, rhythmic shaking that occurs during purposeful actions, such as reaching for a glass, writing, or using utensils. This is a key distinction from Parkinson’s disease, which typically causes resting tremor. In essential tremor, the shaking is triggered by movement, not by stillness.
The precise essential tremor causes are not yet fully understood. Research strongly suggests the condition involves a disruption in communication between specific regions of the brain, particularly the cerebellum. Genetics also plays a substantial role: if a parent has ET, there is approximately a 50% chance their child will develop it as well. While essential tremor is not life-threatening, it is progressive. It typically begins in the hands or arms and can gradually spread to the head, voice, or legs. Recognizing ET as a genuine neurological condition – not simply anxiety or excessive caffeine – is the critical first step toward getting the right help.
When to See a Neurologist for Essential Tremor
Many people dismiss a mild tremor for years, attributing it to aging or stress. But when the shaking begins to interfere meaningfully with your quality of life – affecting your ability to work, causing you to avoid social situations, or making daily tasks genuinely difficult – it is time to seek professional guidance. An experienced essential tremor doctor can make an enormous difference in both your diagnosis and your long-term outcomes.
A specialized essential tremor neurologist is uniquely positioned to help you navigate this condition accurately and confidently. At Lone Star Neurology, our specialists are trained to distinguish ET from other neurological disorders through detailed physical examination and task-based observation. We evaluate how the tremor behaves under different conditions: Does it worsen when you extend your arms? Does it ease when your muscles are fully relaxed? These clinical insights allow us to confirm the diagnosis with precision – ensuring you receive treatment tailored to what you actually have.
How Lone Star Neurology Diagnoses Essential Tremor
Our diagnostic process is thorough, methodical, and patient-centered. We follow the modified Tremor Research Investigation Group (TRIG) criteria – the gold standard for identifying and classifying movement disorders. Our essential tremor doctor team performs a comprehensive neurological examination that assesses muscle strength, coordination, and reflexes, alongside practical task-based assessments such as drawing a spiral, writing a sentence, or holding a cup of water steady.
Differential diagnosis is a central part of our clinical process. We systematically rule out other potential contributors – thyroid dysfunction, vitamin deficiencies, or medication side effects – that can mimic ET. We understand how stressful it is to wait for answers, which is why we prioritize same-day appointments across our DFW locations. Wherever possible, our goal is to deliver a clear diagnosis and a concrete treatment strategy within a single visit.
Essential Tremor Treatment Options
There is no universal solution for ET, but there is a broad and expanding toolkit of effective strategies. For most patients, the first step involves essential tremor medication. Common first-line options include beta-blockers, such as propranolol, and antiseizure agents, such as primidone. These medications work by dampening the aberrant nerve signals that drive involuntary muscle activity. They do not eliminate the tremor, but they can meaningfully reduce its amplitude and restore a significant degree of functional control.
For patients who do not achieve sufficient improvement through oral medication alone, we offer more targeted therapeutic options. One of the most effective is Botox for essential tremor – a procedure that involves injecting botulinum toxin directly into the overactive muscles of the hand or arm. By selectively quieting those muscles, Botox can substantially reduce tremor severity, particularly for patients whose symptoms most affect writing, eating, or other precise manual tasks. Our hand tremor treatment strategies are further supported by occupational therapy guidance and adaptive tools such as weighted utensils, which can make a meaningful practical difference in day-to-day life. In severe cases where both medical and injection-based therapies prove insufficient, surgical options – including Deep Brain Stimulation (DBS) – may also be discussed.
Coming Soon: AbbVie Botox Upper Limb ET Study (August 2026)
Lone Star Neurology is proud to serve as an active center for neurological clinical research. We are pleased to announce an upcoming essential tremor clinical trial launching in August 2026. This study, sponsored by AbbVie (AGN-151607-DP), is specifically designed to evaluate the safety and efficacy of a novel botulinum toxin formulation in individuals with upper-limb essential tremor.
This is a multicenter, double-masked, placebo-controlled study running approximately 72 weeks. We are seeking participants aged 18 to 85 who meet the established diagnostic criteria for ET. Enrollment in a trial of this nature gives patients access to a potentially transformative hand tremor treatment before it becomes broadly available – while simultaneously advancing the science for the wider patient community.
This study is not yet listed on ClinicalTrials.gov, but we are actively building a list of interested candidates now. If standard essential tremor medication has not delivered sufficient relief, this clinical opportunity may represent a significant step forward for you. We encourage you to register your interest through our dedicated research page as early as possible.
